The 5Ds incorporates a multifaceted, personalised approach to mood, sleep, behaviours, addictions/dependencies and circadian rhythms. Circadian rhythms are essential and foundational systems that unify and direct the metabolism of neurotransmitters and hormones.

Dysregulation of this system may play a role in the development of mental health disorders, altered sleep patterns, behavioural disturbance and substance abuse. Moving the clinical approach to the circadian system may help develop treatment strategies for challenging patient presentations and new patients alike.

Almost half of all Australian adults experience a mental health disorder such as anxiety, depression or substance abuse disorder in their lifetime; with 14% of 4-17 year olds experiencing mental health disorders. The 5Ds framework considers the circadian rhythm in the pathogenesis of mental health disorders, alongside Mood Disorder Appraisal results, pathology, medical history and presenting signs and symptoms to categorise patients into five main categories: Driven, Disrupted, Disconnected, Depleted and Dependent.

Once the patient is categorised within the framework it will provide a direction for therapeutic goals and interventions, although not a protocol. The Circadian rhythm is the interface between the external environment and our internal environment and it is linked to our sleep/wake cycle.
